Alluvion Health is a not for profit Federally Qualified Health Center focusing on providing high quality, cost-effective, comprehensive primary and preventive medical, dental and behavioral health care. Alluvion is governed by a Board of Directors comprised of at least 50% consumers and is funded through the U.S. Department of Health and Human Services Bureau of Primary Health Care. This health center is a Health Center Program grantee under 42 U.S.C. 254b, and a deemed Public Health Service employee under 42 U.S.C. 233(g)-(n).
Our mission “Creating and Inspiring Healthier Lives” is a promise we have made to our staff, the community, and our partners. Part of this promise is assessing needs in the community, which is done in a variety of ways. The Community Health Needs Assessment (CHNA) is a tool that is utilized by many partners, including Alluvion Health. This Assessment includes the results of a community survey that measures public perception as well as a variety of other data points at local and regional levels. Resources such as the CHNA as well as direct observation and experience help Alluvion Health identify and meet needs in the community.
