Alluvion’s Projects for Assistance in Transition from Homelessness (PATH) program is a multi-step approach to addressing the housing crisis in our local community and surrounding area. Our team is dedicated to finding long-term housing solutions for those who historically have been unable to utilize outreach opportunities. Being a part of Alluvion Health, the PATH team has access to a plethora of resources to help those who are either currently homeless, or in danger of becoming homeless (particularly for individuals who are suffering from a SDMI (Severe and Disabling Mental Illness) or a co-occurring substance use disorder.

People are eligible for PATH services if they are adults (18 years and older) with a diagnosis of severe mental illness or severe disabling mental illness and an SUD and are experiencing or at risk of experiencing homelessness. Your PATH forward starts here.
More Than Housing
PATH offers services to people who have enrolled voluntarily in the program. Alluvion recognizes that individuals learn, grow, and adapt and differing paces; as such, services are provided at a pace set by each program participant – there is no required timeline. The goal of these services is more than jut finding housing. They address the scope of a program participant’s needs by providing some or all of the following:
- Case management, medication management, and prescription renewal
- Referrals to community behavioral health services, including support services (for example, peer specialist or recovery coach)
- Referral to primary health and dental services, job training, education services, and housing services.
- Screening and diagnostic treatment services
- Habilitation and rehabilitation services
- SUD treatment
- Supportive residential services
- Enrollment in mental health services
- Assistance with Social Security and housing applications and food stamps
