Care Coordinator - Substance Use Disorder (SUD)

$19.00 per hour or more, DOE/DOQ

The Care Coordinator works as a core member of the collaborative team in which the coordinator will bridge the gaps between all departments at Alluvion and with outside agencies. The Care Coordinator will work with the court services counselor with receiving referrals, sending out paperwork, scheduling, and follow up with clients as they move through the levels of care. The Care Coordinator is responsible for coordinating and supporting SUD clients with primary medical care appointments, mental health appointments, dental, MAT, and PATH appointments within the health center and treatment facilities, community organizations, and providing lists of community resources outside of the health care facility. The Care Coordinator provides excellent customer services to patients and providers, while ensuring access to care, assessment for additional services if qualified, and coordination of non-traditional healthcare services to ensure whole patient care is achieved. The Care Coordinator is or will be certified as a Certified Application Counselor to provide outreach and enrollment services for Marketplace insurance and/or Medicaid enrollment. The Care Coordinator is an active participant in clinical team huddles and care planning activities. In addition, the Care Coordinator participates in education and outreach events as needed in the community.
