As an LCPC or LCSW (with LAC credential preferred), the Licensed Clinical Behavioral Health Provider works as a core member of the Collaborative Care Team which involves the patient’s primary care provider and other mental health, addiction or substance abuse providers, as appropriate, to provide comprehensive patient centered health care within Alluvion Health. The Licensed Clinical Behavioral Health Provider is responsible for coordinating and supporting appropriate patient centered mental health care within the parameters of the Health Center’s Patient Centered Medical Home Certification and ensuring services meet appropriate levels of care based on the patient’s diagnosis and need.

Radiologic Technologist
$26.44-30.71 per hour, DOE
The Radiologic Technologist is responsible to take radiologic images under the direction of medical providers. As a member of a comprehensive care team, coordinates with other healthcare professionals, including nurses and providers to ensure continuity of care and appropriate follow-up with patients. Performs patient assessments prior to and during radiologic procedures, preparing the room and the patient for the procedure, which in some cases may involve the oral or intravenous administration of contrast agents in order to improve image quality or obtain specific types of images, and records images. The Radiologic Technologist utilizes radiology equipment accurately, evaluates the quality of the study, while also taking steps to limit radiation exposure on parts of the body that do not require imaging through the use of additional equipment such as protective vests and helmets. In addition, the Radiologic Technologist must effectively communicate with the patient, family, and other healthcare workers as part of an effective patient centered care team.
MEDICAL ASSISTANT - PREVENTION SERVICES

The Medical Assistant (MA) performs routine administrative and clinical tasks to assist in the delivering primary health care to Alluvion Health patients. The Medical Assistant performs clinical support staff procedures along with providing preventive health care education to patients of Alluvion Health, including working with patients of all socioeconomic backgrounds in an effective, culturally and linguistically sensitive manner. Additionally, Medical Assistants will be responsible for making referrals to community services, providing program outreach and performing other duties as assigned.
Phlebotomist
$18.00 – $19.46 per hour, DOE/DOQ
Under the supervision of the Laboratory Manager, participates in specimen collection from all patients including neonates to elderly. This may require travel to other locations as well as to patients’ residence within the city. Provides instruction to patients regarding specimen collection. Must follow established laboratory procedures for collection, labeling and processing specimens throughout the entire testing process. Must utilize the Laboratory Information System (LIS) to order testing, receive specimens and similar functions. Able to work in different locations and Saturdays. May be required to perform waived (CLIA) testing using a variety of specimens. Perform related duties incidental to the work described herein. Actively participates in quality improvement for department efficiency.
