Direct Primary Care is an innovative, alternative, membership-based healthcare model in which you pay a flat monthly fee for unlimited office visits and a wide variety of healthcare services. This is not health insurance and does not bill your health plan for any charges. It can be used by individuals as well as employers looking to offer employees cost-effective, transparent healthcare. Patients receive extraordinary access to providers at Alluvion Health. This healthcare model is available to all ages.

Because of the repeal of the Affordable Care Act, this alternative healthcare model has become a nationwide trend, finding its way into nearly all 50 states. Our goal is to create an affordable opportunity for individuals in our community to seek healthcare services when needed without the burden of significant out-of-pocket expenses. Direct Primary Care provides the opportunity to create change in the overall health of our community, rather than continuing the trend of reactive health treatment. 

Patients receive unrestrictive access to their healthcare provider, report little to no wait time, and receive more one-on-one time with their provider. Walk-in appointments or scheduled appointments are accessible.

As a member of Alluvion Health’s Direct Primary Care program, enrollees gain access to Alluvion Health’s discount pharmacy program. This program is available through the Alluvion Health Pharmacy, and other qualifying pharmacies.

Excluded services are any service not listed under Qualifying Services, including, but not limited to all procedures and appointments outside of Alluvion Health, all lab-work, allergy procedures and serums, and cost of prescription drugs.

MEMBERSHIP PLAN A – Unlimited Access to Medical Visits and Qualifying Services

Individual Pricing
$75 per month
$825 per year if paid-in-full

Family Pricing (family of 4)
$199 per month, $15 per month for each additional dependent
$2,189 per year if paid-in-full

MEMBERSHIP PLAN B – Unlimited Access to Medical and Behavioral Health Visits as well as Qualifying Dental Services (initial dental exam, 2 preventative cleanings per year, one set of annual x-rays) 

Individual Pricing

$125 per month 
Members receive one month free if paid-in-full 

Family Pricing (family of 4)

$299 per month, $20 per month for each additional dependent
Members receive one month free if paid-in-full

Businesses offering Direct Primary Care to their employees will receive a discount based on number of employees enrolled. To be eligible for these discounts businesses must sign an annual contract. Businesses will be invoiced monthly.
